Isaac Johnston, M.D.
Dr. Isaac Johnston is a family practice physician focused on caring for people of all ages. He enjoys working with infants and children, adults and older adults alike. Previously Dr. Johnston provided clinical, hospital and obstetric care. At Spira Care, he helps members manage their acute and chronic concerns, preventive care needs and partner with them to achieve their optimal health and wellness goals. Outside of work, Dr. Johnston enjoys traveling and spending time with his wife and three children. He has a wide range of interests including basketball, playing the piano, golf, running, lifting weights, fantasy football and trivia. Dr. Johnston is originally from Wichita, Kansas.

Diana Faltermeier APRN-C
Diana has over 20 years of nursing experience in emergency medicine, cardiology, plastic surgery and nursing education. She earned her undergraduate nursing degree from the University of Kansas, master’s degree in nursing at the University of Central Missouri and obtained a post-master’s certificate as a family practice nurse practitioner from Maryville University. Diana was awarded the March of Dimes Nurse of the Year in 2013 and has extensive experience in projects to improve patient care and outcomes. Her goal is building trusting relationships to impact and help change lives through lifestyle modifications. Diana’s hobbies include travel, reading, exercise, spending time with friends and family at the Lake of the Ozarks and watching KU basketball.
Amare Haboro, APRN, FNP-C
Amare Haboro is a Family Nurse Practitioner at Spira Care. Amare has worked as a registered nurse at rehabilitation facilities, in long-term care and at Shawnee Mission Medical Center. He spent several years working in critical care nursing where he fell in love with taking care of people during high and low points of their lives. This experience led him to become a nurse practitioner, which is a role that allows him to listen to member concerns and partner with them to form a plan. Amare is passionate about educating members around their medical conditions, medications, treatment plans and procedures, and enjoys working with members of all ages.
Amare earned a Master of Science in Nursing from Graceland University, a Bachelor of Science in Nursing from MidAmerica Nazarene University and an Associate of Science in Nursing from Kansas City Kansas Community College. He is originally from Shawnee, Kansas, and loves the family-oriented nature of Kansas City. In his free time, he plays soccer, watches sporting events, runs, bikes and spends time with his family. Amare speaks English and Amharic.
Stefani Lim, APRN, FNP-C
Stefani Lim is a Nurse Practitioner at Spira Care. Stefani spent five years working in the allergy, asthma and immunology fields. While she remains interested in these fields, she is currently focused on family medicine and preventive care, as well as alternative medicine and integrative health. Stefani strives to arm members with the resources and education they need to live a healthy lifestyle that encompasses all dimensions of mental and physical health.
Stefani was born in Jakarta, Indonesia, and moved Overland Park, Kansas, as an infant. She speaks English and Indonesian. Stefani considers Kansas City to be a hidden gem that offers the ideal mix of midwestern hospitality, manageable traffic and affordable housing. She also appreciates that the city continues to grow in diversity and culture. Stefani is a foodie who enjoys trying new cuisines, spending time with her family, nature, watching movies, dancing and music.

Stephanie Tinsley, APRN-C
Stephanie Tinsley is a Family Nurse Practitioner with more than ten years of experience in patient care. She earned her Master of Science in Nursing from Research College of Nursing. Stephanie decided to become a nurse practitioner after spending several years as a nurse in the emergency department. Now at Spira Care Shawnee, Stephanie has a collaborative relationship with members, in which members play active role in their health care. Stephanie enjoys collaborating with all health care disciplines and is enthusiastic about the team approach of Spira Care.
Outside of member care, Stephanie enjoys spending time with her family and watching her favorite sports teams, including the Royals, Chiefs and Sporting KC. Stephanie’s hometown is Parkville, Missouri.

Sara Nelson-Johns, LSCSW, MSW
BEHAVIORAL HEALTH CONSULTANT
Sara Nelson-Johns is a Behavioral Health Consultant at Spira Care. Sara has 15 years of experience in the mental health field. She has worked and volunteered for organizations and non-profits including Kansas City CARE Clinic, Kansas City Free Health Clinic and Operation Breakthrough. Sara earned a Master of Social Work from The University of Kansas and a Bachelor of Arts in Communications from Bradley University.
